Welcome to myHCV.ca. By using our product, you agree to the following Terms and Conditions of Use ("TAC"). Please read them carefully.
1. TERMS
By accessing this web site, you are agreeing to be bound by these web site Terms and Conditions of Use, all applicable laws and regulations, and agree that you are responsible for compliance with any applicable local laws. The materials contained in this web site are protected by applicable copyright law. We may modify the TAC from time to time. After such change, you will be bound by the modified TAC. We will inform users of any changes, but recommend that you review the TAC periodically.
2. DESCRIPTION OF SERVICE
We provide users with an online health card validation service ("Service"). This Service provides access to Ministry Of Health and Long-Term Care ("Ministry") databases to retrieve patient information online. The materials on myHCV.ca’s site are provided "as is".
We do not provide any medical, financial, legal, or other advice. Validation simply checks the eligibility of the health card number and card holder. It does not guarantee payment of the claim since payment is dependent on additional factors. It does not provide corrected or updated health card numbers or version codes.
Since data obtained is from the Ministry Of Health and Long Term Care databases, we cannot assume responsibility for inaccurate content.
3. SUBSCRIBER FEES
All fees will be paid at the commencement of the Service.
Monthly subscribers to myHCV.ca will pay the Monthly Subscription Fee on a monthly basis. The monthly subscription will begin on the date that the user pays for the Service. The user can be cancel at any time, but will be charged fro a full month.
Annual subscribers will pay the Yearly Subscription Fee on a yearly basis. The yearly subscription will begin on the date that the user begins to pay for Service. The user can cancel at any time, but will be charged for the full year.
4. SUBSCRIBER TERMINATION
The subscription fees will be billed every subscribed time period, from the time you subscribed, at the agreed rates until otherwise terminated. If terminated, the Service will cease at the end of the subscribed time period. After that point, the subscriber will lose access to the site.
Healthcarvalidation.com reserves the right to immediately suspend or terminate a subscription in the event that:
(a) the subscriber fails to pay the agreed amount due to myHCV.ca
(b) the subscriber is found to be using our site for fraudulent purposes
(c) in the event of any conduct by the subscriber that is considered to be in breach of contract.
myHCV.ca will not discontinue any of our subscription products without giving notice to the users.
5. REFUNDS
myHCV.ca does not issue refunds, regardless of the reason for cancellation.
6. AVAILABILITY
The Ministry has scheduled downtime for maintenance every second Sunday of the month between 5:00 a.m. and 8:00 a.m. for their system. Our Service will be unavailable during this time. We are not responsible for any disruptions of service caused by problems in MS EDT or Go Secure.
